Online membership form

Please download and print out the Membership Form (200 Kb PDF document). After filling up the form, please bring it to the next SFS screening along with your payment in cash or cheque. We will then issue your membership card at the door.

Tip -- sign up at the start of the month rather than near the end of the month, to maximize your membership period. Regardless of which day of the month you sign up on, your membership will begin in that month.
